QM board type
One of the questions on the form asks for the "QM board type". Here are photos in case you're unsure which type you have:

The QM32 is about 13" (33cm) long. It fits into an ISA slot.

The newer QM2000 is about 7" (18cm) long. It fits into a PCI slot.

If you need to send your hardware back to Pangolin for repair, please fill out the following form. The data will be e-mailed to Pangolin. This lets us prepare for the board's arrival.
After you click the "Submit" button, you'll go to a page that summarizes the information you are e-mailing. Please print two copies of the confirmation page. Keep one copy for your records, and include the other when shipping your board.

Specifying the shipping address

Pangolin's shipping address (where to send the board) is at the bottom of this page, next to the "P" logo.
Note that the form does not ask for your shipping address. Please be sure that your address is on or inside the package, so we know where to return the board!

This can be found written on the QM board, or by using "Help/About LD" or "Help/About Showtime" in the software. It will be a four-digit number, plus a one- or two-digit suffix: 1234-M, 1500-B, 2001-S1. QM2000 boards may also have a letter between A-H; you can ignore this.

For speed and tracking reasons, the sole method we use to return your board is FedEx. Pangolin pays shipping for in-warranty repairs; out-of-warranty repairs will be billed to you.
